Yes, HIPEC (Hyperthermic Intraperitoneal Chemotherapy) is considered a major surgery. It involves surgically removing visible tumors from the abdomen (cytoreductive surgery), followed by heated chemotherapy delivered directly into the abdominal cavity. The procedure is complex, lengthy, and requires hospitalization and recovery time, typically used for advanced abdominal cancers like mesothelioma or colorectal cancer with peritoneal spread.

HIPEC uses heated chemotherapy drugs, most commonly Mitomycin C or Cisplatin, depending on the type of cancer. These drugs are circulated in the abdominal cavity at high temperatures (around 41–43°C) to enhance their effectiveness. The heat improves drug penetration and kills cancer cells more efficiently while limiting systemic side effects since the chemo stays localized.

HIPEC chemotherapy itself isn’t painful because it’s performed under general anesthesia during surgery. However, post-operative pain is common due to the major abdominal surgery involved.
